SELECTED PLAYERS INFORMATION

Congratulations on your player being selected.

Registration Day for the U8 to U17 divisions will be held on:

Saturday June 8, 2013
from 9:00am to 12:00pm

in the Cooper City Optimist building at the Cooper City Sports Complex, 10500 Stirling Road, Cooper City (behind the police and fire stations)


In order for Registration Day to be successful, please download and complete the forms below:

1.    CCCS - Player Club Agreement * - (Complete for Registration)

2.    CCCS - Medical Release Form * - (Must be notarized and returned)

3.    CCCS - Parent /Spectator Code of Ethics * - (Complete for registration)

4.    CCCS - Player Code of Conduct * - (Complete for Registration)

5.    FYSA - Informed Consent about Concussions or Head Injuries * -(Complete for Registrations)

and bring them with you along with the registration fee of $425.00 for Cooper City resident and $455.00 for non resident made payable to "Cooper City Optimist" and a copy of the player's birth certificate for new players.

Please bring your player with you. We will have uniforms on hand in order to properly size your player for their new uniforms.
